The CMS gave Premier Nursing and Rehabilitation Center an overall rating of 3 out of 5, a figure slightly above North Carolina’s average rating of 2.9 for nursing homes.
Carolina Rivers Nursing and Rehabilitation Center was identified as the second largest facility in Onslow County for the quarter, with 120 available beds.
Premier Nursing and Rehabilitation Center had incurred one fine totaling $51,984 and received one penalty as of the first quarter of 2026.
