City of Jacksonville recently issued the following announcement.
Notice: Due to the evolving event of COVID-19, and out of an abundance of caution, the City of Jacksonville has temporarily suspended all Passport Services.
Passport Services
Business or vacation taking you abroad? The City of Jacksonville is certified through the US Department of State as a Passport Acceptance Facility. We offer the following services:
- Passport forms
- Passport photos
- Expedited and overnight services
- Appear in person
- Completed passport application (DS-11), typed or filled out in black ink
- Proof of US citizenship (birth certificate, expired passport, naturalization certificate)
- Submit proof of identity (driver’s license, military ID, other government issued ID, previously issued passport, etc)
- Provide a recent 2 inches by 2inches high-quality photograph (Jacksonville offers photo service, see below)
- Check or money order for City Services made out to the City of Jacksonville
- Check or money order for USDOS Services made out to the US Department of State. See pricing information below
- Please note that all documents must be original or certified copy
City of Jacksonville Passport Hours of Operation:
City Hall, 815 New Bridge Street
- Walk-ins Monday, Wednesday, Friday, 12-3:30PM
- Appointments Monday - Thursday, 9AM-3:30PM and Friday, 9AM-12PM - Call 910 938-5200 to schedule an appointment.
- Walk-ins Monday - Friday, 8 AM - 7 PM
